Insurance Certificate Request Form

Many Facilities that Al-Anon Groups meet in have been requiring the Al-Anon Groups to provide Proof of Insurance. Our Insurance Provider can do a Certificate of Insurance for these Facilities.

All Certificates of Insurance are sent via email directly to the meeting facility. A copy is also requested to be sent to the group contact person.

Please complete ALL the information in the following form. Missing or Incomplete Information will delay the Certificate being issued.
